New Resources

The CII/CPAR Status and Functions by EMR resource identifies which data types are currently functioning in each EMR. This tool can help users stay up to date as new EMRs and data types continue to be added to CII/CPAR.

As of October 1, 2024, the CPAR provider is being displayed in Connect Care. The CPAR Provider in Connect Care resource explains what it means when the CPAR Provider field in Connect Care does not match the Primary Care Provider field in Connect Care, how document routing is determined and which CPAR provider will be displayed when a patient is on more than one CPAR panels. It also provides guidance on how to update the Primary Care Provider field in Connect Care should a physician be receiving reports about a patient who is not paneled to them in CPAR and is not their patient.

PCPCM Updates

Visit the PCPCM web page for the latest information on this new funding model. The PCPCM Operations Manual is a great tool to answer many questions related to the model and is considered the source of truth. Updates are being made to the website and Operations Manual as more information becomes available. There will be an upcoming Information Session in March (date is TBD) titled “Improving the Accuracy of PCPCM Panels”. This session will be recorded and made available on the PCPCM webpage.

The PCPCM Panel Size Calculation resource explains how AH calculates PCPCM panel sizes, and steps you can take to ensure it’s accurate.

Finally, consider registering for our weekly Q&A sessions that we’re calling PCPCM: Live & on Call with AMA. These sessions are held each Friday from 12:30-1:30pm, and are open to physicians, PCN and clinic staff, and others interested in PCPCM.

Q&A Recap

  1. How will Conflicts be handled in PCPCM for appropriate dual attachments (e.g. maternity care & primary care panel in CPAR)?
    •  The Conflict Working Group is a large multi-organization team that is working on many issues related to Conflicts and PCPCM. They are very aware that appropriate dual attachments do exist and will keep this in mind when considering possible solutions to Conflicts under the new funding model.
  2. Do all EMRs allow for manual resubmissions to CPAR?
    • Some EMRs give users the ability to trigger a resubmission of the CPAR panel after the automatic panel upload has already happened. Resubmissions need to be done before the 20th of the month, or the resubmitted panel will fail to upload. This function is not available in all EMRs.
  3. Are manual panel uploads (i.e. uploading a .csv file to the CPAR Portal) still an option?
    • No, this is not an option for EMRs that are currently conformed for CII/CPAR.
  4. If a physician is not on CPAR, what will be shown in their AMA dashboard under the PCPCM widget?
    • They will see the PCPCM widget, but the PCPCM Panel Size field will be blank.
  5. How often will the AMA dashboards be updated with new PCPCM Panel Size?
    • The AMA dashboards will be updated monthly. AMA relies on reports from AH about PCPCM panel sizes, which can take time to process. The result is that, at least for now, the PCPCM panel sizes displayed in the AMA dashboard are approximately 6 weeks behind the CPAR monthly uploads. We are working with AH on ways to improve the process.

Next meeting is March 18, visit CII/CPAR Connect web page to register.
