Denominator clean-up

The CII/CPAR team sent out denominator clean-up spreadsheets by email to PCN contacts. Many PCNs have completed and returned these spreadsheets to the CII/CPAR inbox – Thank you! If you still have not completed and returned yours, please consider doing this soon as we hope to apply these numbers to the dashboards before the new year!

PCCM panel prep webinar series

The 3-Part PCCM Panel Prep: Take Control of your Panel Size series has now been wrapped up! Thanks to all who attended to learn more about this topic. All the recordings are now available on our website:

Electronic Confirmation of Participation (eCoP)

The eCOP form now has a landing page where you will find instructions on using the form, along with the link to access the form. We continue to encourage anyone onboarding to CII/CPAR to try this new electronic version. If you have feedback to provide on the form, please reach out! 

General updates:

CHR Update

  • The issues preventing Encounters from being turned on for CHR clinics have been resolved, and clinics will start going live for Encounters soon. eHealth and/or Telus will reach out to site liaisons to inform them of a go-live date for Encounters before they are actually turned on. Find some CHR resources related to encounters that may be helpful for your clinics here.

e-Notifications from Connect Care facilities

  • AH has informed our team that they have paused the work to shut off some e-Notifications from Connect Care sites to reduce duplication with Summative Notes. This work will likely resume in the future when other priorities allow.

Allied Health onboarding

  • Allied Health onboarding has been paused for quite a while due to issues with onboarding providers without a PracID. eHealth is working on solutions, but this work has also been put on hold for now due to competing priorities. No timeline for resuming onboarding for allied health professionals has been set.

Certificates of Achievement!

Congratulations to St. Albert Sturgeon and Highland PCNs who received their Certificates of Achievement in reaching the goal of 80% participation in CII/CPAR! There are at least 5 more eligible PCNs in the North and Central zones and we are working to get those certificates out to those teams! Once the denominator clean-up is complete by January 2025, there might be many more PCNs identified as having reached this milestone too, so stay tuned!

Q&A recap

  • Why is a physician receiving notifications from hospital when the patient is no longer on the CPAR panel: When the physician is listed in Connect Care as the primary care provider they will continue to receive summative notes, which can look similar to an e-notification in the EMR. To have the physician removed from the primary care provider field in Connect Care, you can contact the AHS Solutions Centre at 1-877-311-4300. Reminder: depending on what scheduling software you use, the new Connect dates that you’ve registered for may not be automatically added to your schedule. After registering, check your email for confirmation of registration. This email should include an .ics file. Open this file, choose your scheduling software (e.g. Outlook), and import the meeting series into your schedule to ensure you don’t miss any! You should already have monthly Connect meetings in your schedule until April 2025. If you haven’t registered yet, click here.

What is CII/CPAR Connect?

CII/CPAR Connect is a regular meeting for anyone needing support with CII/CPAR and aims to support those implementing CII/CPAR by creating a forum to connect and ask questions.

 

Meetings run monthly, typically on the third Tuesday of the month at 1 p.m. You can choose to attend any one meeting or all of them.
