Fall Practice Facilitation Core Training – Registration Now Open!

Practice Facilitator Core Training is designed to support those working directly with clinic teams to build facilitation skills and drive meaningful change in primary care. Sessions are available in Calgary and Edmonton, with virtual and in-person components that allow participants the opportunity to apply learnings at clinics in real time. There is no cost to register.

Registration Deadlines:

  • Edmonton: October 1, 2025 with sessions running Oct 8 – Jan 21, 2026
  • Calgary: October 2, 2025 with sessions running Oct 9 – Jan 29, 2026

The Power of Teams: Navigating Scopes of Practice in Health Care Teams

H2H2H: Transitions in Care Talk series

AMA-ACTT is hosting another webinar in the Transitions in Care learning series in partnership with Primary Care Alberta. These free webinars are open to anyone interested in improving patient transitions from home to hospital to home.

Notification to Change Primary Provider 

Evidence suggests that health outcomes are improved when patients have an established relationship with a family physician who quarterbacks their care. Managing CPAR conflicts strengthens those relationships and is a foundational step towards building a stronger Patient’s Medical Home.


The Notification to Change Primary Care Provider resource helps clinics manage CPAR Conflicts. Clinics can send a fax or letter to providers or other clinics, requesting they remove a patient from their panel. 


Clinics can download and customize the template as needed, including integrating the document into their EMR as a letter template or macro.

Reducing Panel Conflicts

Community Information Integration & Central Patient Attachment Registry (CII/CPAR) adoption among family physicians has reached an all-time high.
