Denominator Clean-up

The CII/CPAR team is organizing a denominator clean-up for all PCNs for late 2024 or early 2025. Once the data is collected from each PCN it will be applied to the CII/CPAR dashboard. PCN staff can expect to receive a list of physicians and accompanying instructions from AMA-ACTT to assist with the clean-up. Please refrain from sending in lists to the CII/CPAR inbox for denominator clean-up as our team is unable to accommodate ad hoc updates.

Update: PCCM Panel Prep Webinar Series

Physicians and teams are invited to join AMA-ACTT for Part 3 of this webinar series and learn what practical steps can be taken to improve the accuracy of their AH panel size estimates.

 

Improving panel size accuracy may increase physicians’ future PMSP payments, which are recalculated and paid out quarterly. Ensuring panel sizes are accurate may also improve payments under future compensation models or funding initiatives. Register for Part 3 today, and please distribute the registration link and details to family physicians and their teams that you support.

Practice Facilitation (PF) Network: Panel Prep Webinar

At the October 7 PF Network webinar, ACTT provided an overview of how the Panel Management Support Program (PMSP) panel size is calculated, and what steps you can take to help your paneling providers get more accurate panel size estimates. The presentation was a summary of the content covered in all three parts of the physician-focused “PCCM Panel Prep” webinar series but tailored to Practice Facilitation roles. Electronic Confirmation of Participation (eCoP)

Please use this link to access the eCoP. ACTT would like to encourage those submitting CoPs to give this electronic version a try.

During this early launch phase, once a form is submitted, please email the CII/CPAR team at ciicpar@albertadoctors.org to notify us that you have submitted a form. This will ensure we have received the eCoP and that our system is functioning properly.

Reminder: The form is saved with each “Next” click and you can use the emailed link to resume editing in the future. You can even forward the link to other colleagues or physicians that may need to provide input on the form.

CII Development Updates

CII can now upload Pulmonary Function Diagnostics/ Tests (PFDs/ PFTs) to the investigations folder in Netcare. Currently, only Healthquest is conformed for this process. Please notify the CII/CPAR team if you are aware of any clinics interested in submitting PFDs from any CII-conformed EMRs.

Starting this November, a few clinics using Healthquest will start testing a new “Patient Summary” function of CII. Other EMR vendors will eventually be working at implementing the Patient Summary as well. Stay tuned for updates.

Q&A recap

  1. If 2 or more CPAR providers are listed in Netcare, which provider will populate in Connect Care “Other Patient Care Team Members” and which provider will receive notifications from Connect Care?
    Only the first CPAR provider listed in Netcare will populate in the “Other Patient Care Team Members” field in Connect Care. When 2 or more providers are listed in Connect Care notifications will only go to the provider indicated in the “Primary Care Provider” field in Connect Care. This field is populated through manual entry and does not pull information from Netcare or CPAR.
  2. Can the eCoP be printed?
    Currently, there is no way to print the eCoP. The CII/CPAR team has taken this feedback, however in the meantime please consider saving the email with the unique link or bookmarking the link for each eCoP you submit.
  3. What incentives are there for pediatricians to go live on CII/CPAR?
    The CII/CPAR Acceleration Grant is presently the main financial incentive for pediatricians, in addition to the improved care continuity for their patients!
  4. How are errors with date of birth resolved?
    If a demographic error exists in Netcare and is correct in the EMR, the patient will need to complete and submit a form or go to an Alberta Registry to have the error corrected. Some changes in demographic will require supporting documentation.
    If the error exists only on the Demographic Mismatch Report and is correct in the EMR and in Netcare, please email ehealthprovidersupport@gov.ab.ca and copy ciicpar@albertadoctors.org.
  5. Should a new Primary Care Provider wait until they have seen 500 patients to sign up for CII/CPAR?
    No, the panel size of 500 is a minimum panel size for eligibility related to the Panel Management Support Program and should not be used to guide any clinical or practice decisions. CII/CPAR improves care continuity and patient attachment at any panel size so there is no reason to wait to sign up for CII/CPAR while building up a practice.
  6. Is eHealth reaching out to clinics to correct demographic mismatches?
    Yes, eHealth may reach out to clinics who are only uploading encounter data and consults (i.e. those utilizing CII only) as these clinics are not able to access demographic mismatch reports through the CPAR portal. eHealth is providing lists to these clinics instead in order to correct demographic mismatches and have information such as consult reports flowing to CII correctly.

What is CII/CPAR Connect?

CII/CPAR Connect is a regular meeting for anyone needing support with CII/CPAR and aims to support those implementing CII/CPAR by creating a forum to connect and ask questions.

 

Meetings run monthly, typically on the third Tuesday of the month at 1 p.m. You can choose to attend any one meeting or all of them.
