Milestone Celebration!

Dr. Ernst Greyvenstein – Family Physician, AMA-ACTT Medical Director, and long-time CII/CPAR champion – came to this week’s Connect to announce that 86% of Primary Care Network physicians are now live on Central Patient Attachment Registry (CPAR) and to thank everyone that helped make this possible.

In less than seven years adoption has grown from just a few test clinics to more than 850 clinics across Alberta. This eclipses the 80% target set early in the project and benefits millions of patients by increasing their continuity of care. This incredible achievement was made possible by the collective efforts of physician leaders, PCN leadership and staff, eHealth/Alberta Health, AMA, AMA-ACTT, EMR vendors, participating physicians and their teams and many others. 

And thank you to those who participated in our “CII/CPAR Share your Knowledge Game”!  It was wonderful to hear stories about past and present CII/CPAR wins!  It was great way to reminisce and celebrate how far we have come!

New Resources available

  • CII/CPAR Status and Functions by EMR - this document will help teams identify which data types are currently functioning in each EMR. These statuses may change over time as EMRs develop, change or add new data types.
  •  PCCM Panel Size Calculation - this document can help physicians and teams to better understand the criteria that AH uses to calculate PCCM panel sizes. Physicians can view their PCCM panel size on their AMA Member Dashboard. Please visit the PCCM website for the latest information on the new funding model. There is an upcoming Information Session for physicians to attend on Jan 29, 2025.

Q&A Recap

  1. Can Long Term Care patients be paneled in the EMR so that they are added to a physicians CPAR panel? 
    Physicians seeing patients in long-term care are likely charting a different systems for these patients than their clinic EMR. If physicians consider themselves to be the primary provider for a long-term care patient, they may choose to create a chart for these patients in their clinic EMR. The clinic would need to have processes in place to ensure these patient charts meet the criteria for CPAR panel inclusion, which can be more difficult for patients that don’t attend appointments at that clinic location.
     
  2. How does the Accuro populate the last appointment date field?
    In Accuro, navigate to Search Patient > F4 > Appointment History. The most recent appointment date shown will the uploaded to CPAR as the date of last visit.
     
  3. Physicians are reporting discrepancies in the EMR to CPAR to PCCM panel calculations, what support can teams provide with this? 
    Physicians and teams are encouraged to visit the PCCM | AMA webpage for information and upcoming information session to stay up to date on this new funding model. The PCCM Panel Size Calculation resource can help determine how these panel sizes are calculated. We are aware of some data entry issues that resulted in missing or incorrect panel sizes in the dashboard. AMA-ACTT is working with AH to correct these issues.

  1. What is the process for terminating a panel for a provider who has retired or moved away from a clinic if the panel termination form was not signed by the provider? 
    If a signature was not received for a Panel termination form due to a provider having already left the clinic, proceed with submitting the form to eHealth, and make them aware that you were unable to obtain the signature from the provider involved. eHealth will be able to assist with proceeding to terminate the panel.

CII/CPAR Connect is a regular meeting for anyone needing support with CII/CPAR and aims to support those implementing CII/CPAR by creating a forum to connect and ask questions.

 

Meetings run monthly, typically on the third Tuesday of the month at 1 p.m. You can choose to attend any one meeting or all of them.
