Reference Interval Changes for Glucose in Neonates
Effective September 4, 2024, the lower age range for fasting and random glucose reference intervals will change from less than 30 days to less than 72 hours. The lower end of the less than 72 hour reference interval will be adjusted from 2.5 mmol/L to 2.6 mmol/L.

Transitions in Care Measures
H2H2H Transitions Monitoring Measures help teams assess and plan their transitions in care work. These measures can be used as potential indicators to assess several components of the H2H2H Transitions Guideline across the patient journey.  Alberta Health Services (AHS) measures key monitoring indicators, including:
  • The number of patients who identified their primary care provider (PCP) during their hospital stay.
  • The number of discharge summaries completed by acute care teams within 24 hours, including those documenting the patient's LACE index.
  • The number of moderate- and high-risk patients, with and without a documented primary care provider (PCP), who receive follow-up care within the recommended timeframes.
  • The number of high-risk patients who did or did not receive follow-up care within the recommended timeframes and were subsequently readmitted to hospital.

Recovery Alberta
Designed with the help of Albertans, the new Recovery Alberta website ensures that patients and Albertans have comprehensive information about Recovery Alberta. This web presence will help increase awareness for Recovery Alberta, and build trust with Albertans by providing reliable information about mental health and addiction and correctional health services

The launch of this website is the first phase of many, as AHS transitions existing addiction and mental health and correctional health services information to the Recovery Alberta website. It will continue to expand to accommodate the evolving needs of the organization and Albertans.
