Transitions in Care Talk – System Navigation in Indigenous Communities

Join us on January 27 from noon to 1 p.m. for the seventh webinar in the Home to Hospital to Home Transitions in Care Talk series hosted by AMA-ACTT and presented by Primary Care Alberta.  


This session explores how navigation supports can foster safe and supportive care transitions for Indigenous individuals, families and communities.

Proof in Practice: Showcasing Our Progress in the Province at IHI

The international spotlight shone brightly on two AMA-ACTT-led Alberta health care projects at the Institute for Healthcare Improvement (IHI) Forum in December.


Our team shared stories of enhancing patient access to timely care and establishing primary care compensation reform in Alberta, highlighting how evidence-based change management, physician co-design and data-driven insights are shaping a more sustainable and equitable system. 


ACTT's presentation on Alberta’s Primary Care Physician Compensation Model (PCPCM) sparked meaningful discussion that reinforced how closely Alberta’s work to modernize primary care aligns with international efforts.


Physicians from the Specialty Care Alliance, the Section of Family Medicine and the PCN Physician Leads Executives were also in attendance, and the event provided a great opportunity to recognize the impactful work happening in our province and the physicians, clinic teams and ACTT staff who make it possible.

Winter Practice Facilitation Core Training: Comprehensive – Now Open for Registration

Are you or someone on your team leading quality improvement work?

Our AMA-ACTT Practice Facilitator Core Training: Comprehensive course is designed to support people who guide quality improvement work with clinic teams. This comprehensive and practical training helps participants build the facilitation skills and quality improvement knowledge needed to support meaningful change.


This winter, we’re offering Comprehensive Training sessions in Calgary and Edmonton. These sessions include both virtual and in-person components, giving participants the opportunity to apply their learning in real time.


Registration Deadlines:

  • Edmonton: February 4, 2026
  • Calgary: February 5, 2026
